INFORMATION CLAUSE
In connection with the information obligation resulting from Art. 13 sec. 1 and 2 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46 / EC (hereinafter referred to as “GDPR “), The Warsaw University of Life Sciences informs:
- The administrator of your personal data is the Warsaw University of Life Sciences, ul. Nowoursynowska 166, 02-787 Warsaw, hereinafter referred to as the “Administrator”. You can contact the Administrator in writing by traditional mail by writing to the address of our registered office indicated above.
- The administrator has appointed a Data Protection Officer with whom you can contact in all matters related to the processing of your personal data and the exercise of your rights under the GDPR, in the following way: in writing by traditional mail, writing to the following address: ul. Nowoursynowska 166, 02-787 Warszawa or via the e-mail address: iod@sggw.pl.
- Your personal data will be processed:
- in order to take action before concluding the Agreement for the provision of educational services and to perform the Agreement for the provision of educational services to which you are a party, hereinafter referred to as the “Agreement” – pursuant to art. 6 (1) (a) b) GDPR;
- in order to fulfill the legal obligations incumbent on the Administrator in connection with the implementation of your education process, including documenting the course of studies, the Administrator’s performance of reporting, archival, statistical, accounting, accounting obligations, carried out, among others Others. based on the provisions of the Act on Higher Education and Science and the implementing acts issued on its basis, the Act on the national archival resource and archives, the Act on public statistics – pursuant to Art. 6 (1) (a) c) GDPR;
- in order to secure and pursue any claims arising from the Agreement or to protect against such claims and for direct marketing, as legitimate interests pursued by the Administrator – pursuant to art. 6 (1) (a) f) GDPR;
In the event that the processing of personal data for purposes and to the extent that goes beyond the cases described above, will require your consent, we will ask for such consent each time, indicating separately each purpose for which your personal data will be processed. personal data.
- Your personal data will be kept for the period necessary to implement the education process, including the implementation of the rights and obligations under the Agreement for the provision of educational services, and after that time for the period necessary to secure and pursue any claims under the Agreement or to protect against such claims and for archival purposes for a period of 50 years. In the event of processing your personal data on the basis of your voluntary consent, the Administrator will store them until you revoke the above-mentioned consent for their processing.
- You have the right to access your personal data, rectify it, delete or limit processing, transfer personal data, the right to object to data processing.
- If the processing of your personal data is based on your consent, you have the right to withdraw your consent to the processing of personal data at any time. Withdrawal of consent does not affect the lawfulness of the processing which was carried out on the basis of consent before its withdrawal. Withdrawal of your consent will be possible by submitting an appropriate statement via the e-mail address: iod@sggw.pl or in another form indicated by the Administrator, about which you will be informed / not informed no later than at the time of expressing such consent.
- You have the right to lodge a complaint with the supervisory body, in the territory of the Republic of Poland this body is the President of the Personal Data Protection Office.
- More information on the possibilities and manner of exercising your rights, described above, can be obtained by contacting the Data Protection Officer in the manner specified in point 2 above.
- Providing your personal data by you is voluntary but necessary to achieve the purposes for which they are collected by the Administrator. If you do not provide your data, it will not be possible for the purposes for which they are collected and indicated above.
- The recipients of your personal data will be or may be those entities to which the Administrator is obliged to disclose them on the basis of generally applicable provisions of law (e.g. public authorities), as well as entities to which the Administrator entrusts the processing of personal data on the basis of a contract in connection with the provision of to the Administrator with services that are necessary for the proper implementation of the education process, including the performance of the Agreement (including IT system operators, payment system operators, entities providing hosting, consulting, auditing, legal, accounting and postal services).
- Your personal data will not be subject to automated decision making and will not be profiled.
